A case of pan-resistant Klebsiella pneumoniae infection in an extremely premature infant treated with polymyxin B sulfate

Abstract: Resistance of Klebsiella pneumoniae to antimicrobial drugs is on the rise in neonates[1], and timely and rational selection of effective antimicrobial drugs is crucial. As the detection rate of carbapenem-resistant Gram-negative bacteria increases, the use of polymyxin, as one of the last lines of defense against carbapenem-resistant bacteria, has been increasing in recent years [2-3]. Polymyxin B sulfate is rarely used in infants and young children due to nephrotoxicity, neurotoxicity and other adverse effects, so the clinical data on its use in preterm infants is very limited. This case report describes a case of carbapenem-resistant Klebsiella pneumoniae-associated early-onset sepsis in a very premature infant admitted to our hospital. After rational application of polymyxin B sulfate, the outcome of the treatment was reversed, and the patient was discharged from the hospital successfully.
